Treating Wrinkles – A Systematic Approach

Maintenance

  • Sun Exposure / Tanning Booth – damage from UV radiation combined with the dry winds of California will lead to early aging and wrinkles
  • Start Young
  • Sun block – I use a high SPF, with a high amount of Zinc Oxide. Apply it every few hours when in the sun. Wear a hat and try to stay in the shade
  • Skin Care Regiment – Begin our treatment plan and keep your skin looking young and glowing

Faint Wrinkles (Age 20’s, 30’s, 40’s)- Our Treatment

  • Our Skin Care Clinic – Complete skin evaluation and a skin care maintenance program designed specifically for you, consisting of the application of mild peeling agents and home skin care products
  • Microdermabrasion – I have found this to be of little value, because we have lower cost skin care treatments that provide better results
  • Lasers and Deep Peels – I avoid these treatments for faint wrinkles because they are too aggressive for younger skin

Fine Wrinkles (Age 30’s, 40’s)- Our Treatment

  • Our Skin Care Clinic – Complete skin evaluation and a skin care maintenance program designed specifically for you, consisting of the application of mild peeling agents and home skin care products
  • Botox or Dysport – a good addition to keep deep wrinkles from forming in the forehead, frown area and around the eyes
  • Lasers and Deep Peels – are still not necessary at this stage
  • IPL / Intense Pulse Light – I do not use this treatment, because we have lower cost treatments available that are as effective

Moderate Wrinkles (age 30’s to 50’s)- Our Treatment

  • Our Skin Care Clinic – Complete skin evaluation and a skin care maintenance program designed specifically for you, consisting of the application of mild peeling agents and home skin care products
  • Botox or Dysport – to lessen your wrinkles and keep deep wrinkles from forming in the forehead, frown area and around the eyes
  • Restylane – Perlane – Radiesse – Fat injections – these soft tissue fillers are very effective for Moderate Wrinkles and are a good choice to maintaining a youthful appearance
  • Moderate Depth Peels – such as a 35% TCA is a great freshening peel that removes moderate and fine wrinkles and corrects blotchy pigmented skin
  • Blue Peel by Obagi – this is a TCA peel with a color agent designed to help those new at doing chemical peels decide when to stop applying the chemical. Experienced Plastic Surgeons will not need to use this added agent
  • Blue Peel without anesthesia – this is done in layers or levels and designed so nurses and others can perform the procedure
    • I do not use any Peels without anesthesia for Moderate Wrinkles, because the depth the chemical needs to penetrate to have an effect will cause too much pain
  • 70% Glycolic Acid Peel – I do not use this because the TCA Peel gives more consistently good results
  • Lasers Ultra-pulsed CO2 and Erbium:YAG – I like to use the Laser for Moderate Wrinkles, but more often a 35% TCA Peel will be a better choice. The CO2 laser will give more tightening of the skin and would be my choice over the Erbium:YAG
  • Thermage / Radiofrequency / RF- once touted as the non-surgical replacement for a Face Lift most people who have had the treatment will tell you they experienced no results
    • I do not use or recommend Thermage, because of its ineffectiveness

Deep Wrinkles (age 40’s to 80’s)- Our Treatment

  • Our Skin Care Clinic – Complete skin evaluation and a skin care maintenance program designed specifically for you, consisting of the application of mild peeling agents and home skin care products
  • Laser – I would use the Ultra-pulsed CO2 laser which gives maximum skin tightening and correction of the wrinkles
  • Phenol / Croton Oil Deep Peel – This would be an option on extremely deep wrinkles, however the facial pigment loss can lead to very light skin color
  • Thermage – as mentioned above, I would avoid this skin treatment
  • Face Lift – to get the maximum results, I might suggest you think about Facial tightening prior to treatment of your skin
  • Botox or Dysport – to soften and lessen the appearance of wrinkles
  • Restylane – Perlane – Radiesse – Fat injections – these soft tissue fillers are less effective for deep wrinkles, but still are used
